*If you have difficulties with contacting a teacher over the summer you may have a coach or adult figure (ex. All-star Coach, soccer coach, dance teacher) write you a reference instead. NO FAMILY MEMBERS! PLEASE try to get a teacher recommendation first.

IT IS YOUR RESPONSIBILITY TO COLLECT THE SEALED AND SIGNED ENVELOPE FROM YOUR TEACHER AND RETURN IT TO COACH TARLTON. YOU MAY RETURN THIS FORM PRIOR TO TRYOUTS AT ANY OF THE SUMMER PRACTICES LISTED ON THE MYERS PARK WEBSITE. YOU MAY ALSO RETURN IT TO COACH TARLTON’S BOX AT THE FRONT OFFICE OR YOU MAY MAIL IT. REMEMBER YOU NEED TWO RECOMMENDATIONS!

Thank you for your assistance in evaluating potential squad members for the 2008-2009 cheerleading season. Please rate the student as indicated; upon completion, please return this form directly to the student in the envelope provided. The envelope should be SEALED AND SIGNED. If you would prefer returning this form to me directly you may mail it (see address above) or you may place it directly in my box at the front office (labeled: Cheerleading/Yasmiin Tarlton). Tryouts are not until the end of July, yet please try and return this form as soon as possible. All responses are confidential. Thank you again for your assistance!
